Friday, November 9th 1:00 - 2:30 PM, The Black Rock Branch Library, 2705 Fairfield Avenue presents: Coordinated Assessment and Referral for the Elderly The CARE program is a Coordinated Assessment and Referral for the Elderly in the Fairfield county. The assessment & navigation services seniors receive from the CARE program social worker are FREE. The CARE program social worker visits the community regularly, and targets those rooted in areas that are of great need of direction to get connected to appropriate services for safe community living.
